FTPW Registration Info

WELCOME TO THE 2019-2020 FTPW SEASON!!!!

We are excited about our 2019 Season Opening! As the season begins, there are a few things we would like to communicate so that we can have a smooth transition into the year.

Registration

No Refunds will be givien after August 9, 2019

Early Bird Special (March 31- May 31) -$65 Flag $125 Tackle

Regular Registration (June 1st – July 31st) - $150 Tackle $65 Flag

In-Person Registration Dates:

·          June 22nd at Middle bush Park concession Stand 10am -1pm

·         July 13th at Middle bush Park concession Stand 10am -1pm

·         July 20th at Middle bush park concession Stand 10am -1pm

Sibling Discount - $25 off per additional child

PLEASE BRING THE ATTACHED FORMS COMPLETED FOR YOUR CHILD (REN)'S 2019 REGISTRATION:

Forms must be downloaded and printed from www.ftpw.org (located under forms section) forms are required to be completed for both in person and online registration.

  1. 2019 Participant Contract and Parental Consent form

  2. 2019 Physical Fitness & Medical History Form (form must have original doctor’s signature/stamp)

  3. Parental/Guardian Web Site & media Publication Consent Form

  4. Emergency Contact Form

  5. Volunteer Requirement Form

  6. Franklin Warriors Player Contract

  7. Current Child Photo (head shot)

  8. 2 copies of final 2019 report card (all 4 marking periods)

  9. Original Birth Certificate and a copy

 

Fundraising

  • Fundraising MUST BE PAID IN FULL AT THE TIME OF IN-PERSON REGISTRATION (NO EXCEPTIONS): NO PERSONAL CHECKS. We will ONLY ACCEPT CASH, MONEY ORDER, or CREDIT/DEBIT CARD as form of payment.

     

  • Fundraising raffles are only available during in-person registration and is a requirement of all families.

     

  • When registering online you will be automatically be charged the fundraising fee of $150, you can then pick up raffle tickets when in person registration is taking place. When you complete your 5 hrs, you get $50 back from fundraising. 

 

**VOLUNTEERS ARE NEEDED FOR REGISTRATION & DURING THE SEASON: ALL VOLUNTEERS HOURS WILL GO TOWARDS THE REQUIRED 5 HOURS PER FAMILY**
